Before leaving the hospital, ask for a written follow-up plan. It should identify the treating team, the next appointment and the contact for questions about your care.
Collect your discharge summary, procedure information, investigation results and medicine instructions. Ask what should be shared with your clinician at home.
Clarify whether follow-up can happen remotely, what it costs and which situations require an in-person visit. Check the time zone and communication method for remote appointments.
Ask your treating team to explain symptoms that need urgent attention and where to seek help. A coordination service does not replace local emergency care.
